With the recent launch of the Partnership for Patients initiative, there has never been a better time to put tools to work for your hospital that can help to track and eliminate adverse events. The initiative has set an ambitious goal for all U.S. hospitals: reduce preventable all-cause harm rates by 40% in organizations by the end of 2013. The Partnership for Patients website says that to succeed, organizations will need to “begin tracking and targeting harm in ways most hospitals have never attempted—by examining all types of patients, and for all types of injuries and then, wherever harm is found, asking: What can we do to prevent it from happening again?”
